Coverage Note:
You must be enrolled with one of these health plans to access CalAIM Community Supports through Connections Care Home Consultants.
SOC Threshold:
Share of Cost is usually triggered if a member receives more than $1,800/month, although this can vary by county and circumstances.
SNF Residents:
Members in Skilled Nursing Facilities with income above $1,800/month may not show a SOC since the SNF receives most income. This may change when transitioning to community living.
